This competence is about working directly with older people and their
carers to develop and agree individualized care plans to reduce the risk of falls. It covers agreeing the goals and component parts of the care plan, and identifying services or agencies which are appropriate and available to provide the required interventions.

In the context of this competence older people are those with a history of recurrent falls or those assessed at increased risk of falling. In line with best practice, individualized care plans for older people at risk of falls involve multi factorial intervention programs focused on the individual’s particular risk factors. Users of this competence will need to ensure that practice reflects up to date information and policies.
